Key Responsibilities:
- Manage and provide direct patient care, ensuring a holistic approach that encompasses psychosocial, spiritual, cultural, and physical needs.
- Coordinate care efforts among team members to meet the diverse needs of patients and families efficiently and effectively.
- Advise on alternative care strategies while adhering to established policies and procedures.
- Educate team members on symptom management, case coordination, and available resources.
- Enhance the clinical capabilities of team members by serving as a mentor and resource.
- Collaborate with various departments to address patient care requirements comprehensively.
- Communicate effectively with all stakeholders, providing constructive feedback and resolving conflicts.
- Ensure prompt resolution of customer concerns, delivering exceptional service to patients and families.
- Conduct reviews of patient care documentation and provide clinical supervision to identify strengths and areas for improvement.
- Perform additional duties as assigned.
Qualifications:
- Current RN license in the relevant state or multistate/compact license.
- At least two years of nursing experience.
- Experience with Electronic Medical/Health Record systems.
- Certification from HPNA within two years of assuming the role.
- Current BLS certification for healthcare professionals.
- Valid driver's license and insurance as per company policy.
- Reliable transportation for patient visits.
- Strong critical thinking and problem-solving abilities.
- Effective communication skills in English.
- Flexibility in scheduling to accommodate patient needs.
- Ability to navigate various home environments safely.
- Capacity to manage the emotional demands of working with terminally ill patients and their families.
- Physical capability to perform essential job functions, including lifting and mobility tasks.
