Job Description

We are seeking a skilled and compassionate Nurse to join our team as a Home Health Visiting Nurse. As a key member of our interdisciplinary team, you will provide intermittent skilled nursing services to patients in their homes, working closely with physicians and other healthcare professionals to ensure seamless care coordination.


Key responsibilities include:

  • Assessment and Evaluation: Conduct thorough assessments of patient needs and develop individualized care plans to address identified problems and goals.
  • Clinical Judgment: Use sound clinical judgment to make decisions regarding patient care, communicating effectively with patients, families, and healthcare teams.
  • Documentation: Accurately document patient information, including progress notes, treatment plans, and outcome evaluations.
  • Communication: Maintain open lines of communication with patients, families, caregivers, and healthcare professionals to ensure effective care coordination and delivery.

Required Skills and Qualifications

To be successful in this role, you must have:

  • A valid State professional Nurse License.
  • Excellent verbal and written communication skills.
  • Proficient clinical skills, including assessment, evaluation, and documentation.
  • Able to work independently and as part of an interdisciplinary team.
  • A strong commitment to providing high-quality patient care and improving health outcomes.
