About the Role

We are seeking a skilled Licensed Practical Nurse to join our team at Alumus. As a Patient Care Specialist, you will play a vital role in providing high-quality care to our patients in the comfort of their own homes.

Key Responsibilities
  • Utilize the nursing process to provide optimal quality and continuity of care to patients.
  • Complete and scan required record-keeping forms/charts upon patient admission, transfer, and/or discharge.
  • Chart nurses' notes in an informative and descriptive manner that reflects the care provided to the patient, as well as the patient's response to the care.
  • Report all discrepancies noted concerning physician's orders, diet change, charting error, etc., to the Director of Nursing.
  • Ensure that appropriate documentation concerning unauthorized discharges is entered in the patient's medical record in accordance with established procedures.
Requirements
  • Current, unencumbered LPN/LVN license required or Compact State license required.
  • A minimum of 2 years LPN/LVN experience required.
  • Able to educate patients and family on multiple disease processes and have good medication knowledge and wound care experience.
