About the Role
CarePartners is seeking a skilled Physical Therapist to join our Home Health Franklin Team. As a Physical Therapist, you will play a vital role in providing high-quality, patient-centered care to individuals in their homes. This is an excellent opportunity to make a meaningful difference in the lives of our patients and their families.
Key Responsibilities
- Conduct comprehensive assessments and develop individualized plans of care to meet the unique needs of each patient.
- Provide skilled physical therapy services, including exercises, modalities, and education, to promote optimal patient outcomes.
- Collaborate with the interdisciplinary care team to ensure seamless coordination of care and effective communication with patients, families, and healthcare providers.
- Document patient progress and outcomes in accordance with regulatory requirements and CarePartners' policies.
Requirements
- Current North Carolina license to practice Physical Therapy.
- Driver's license required.
- BLS/CPR Certification.
- One to two years of related experience.
