Physical Therapist CarePartners Home Health Specialist

About the Role:
CarePartners is seeking a dedicated Physical Therapist to join our Home Health team. As a Physical Therapist, you will play a vital role in providing high-quality patient care and improving outcomes for our patients.
Job Summary:
The Primary Physical Therapist in Home Health will manage an assigned caseload of patients, providing therapy and clinical case management to improve patient outcomes and manage resources effectively. This role requires a comprehensive assessment/examination/evaluation, development/modification of a plan of care in conjunction with the patient/caregiver and physician, and provision of treatment visits and discharge planning to facilitate patient/caregiver engagement for achievement of patient goals, clinical outcomes, and positive patient experience.
Responsibilities:
- Conduct comprehensive assessments and evaluations to identify patient needs and develop individualized plans of care.
- Provide physical therapy services to patients in their homes, including exercises, education, and equipment training.
- Collaborate with the care team to ensure seamless transitions and coordination of care.
- Communicate effectively with patients, caregivers, and healthcare providers to ensure optimal patient outcomes.
- Document patient progress and outcomes in accordance with regulatory requirements and CarePartners policies.
Requirements:
- Current North Carolina license to practice Physical Therapy.
- Driver's license required.
- BLS/CPR Certification.
- One to two years of related experience.
- Preferred experience in acute care procedures, especially orthopedics, neurology, cardiopulmonary, and care of burns/open wounds.
