Job Title: Licensed Clinical Social Worker

Reimagine Care is a pioneering company revolutionizing oncology care by providing innovative, technology-enabled services to patients and healthcare providers. We seek a highly skilled Licensed Clinical Social Worker (LCSW) to join our Virtual Care Center team.

Role Overview

The LCSW will work closely with our clinical team to provide comprehensive psychosocial care to cancer patients and their families. This includes assessment, counseling, education, advocacy, and support programs to enhance patients' ability to cope with physical, emotional, environmental, and social problems.

Key Responsibilities
  • Provide psychosocial assessments, treatment planning, and interventions or referrals for oncology patients with social, environmental, emotional, financial, and/or medical needs.
  • Deliver individual counseling using cognitive behavior therapy (CBT) and recognize, treat, monitor, and refer to appropriate resources when patients experience distress throughout the disease trajectory.
  • Facilitate patient and family/caregiver support groups and provide referrals to resources such as financial, spiritual, social, and emotional support programs.
  • Respond promptly to patient/family and care team needs, documenting appropriately.
Administrative Responsibilities
  • Assist in the development and implementation of programmatic planning and standards of practice.
  • Monitor selected clinical indicators and departmental audits.
  • Work with the Senior Director of the Virtual Care Center to establish and manage volume/productivity targets.
Requirements
  • Active social work license at the Licensed Certified Social Worker (LCSW-C) level.
  • Graduate of a Master's degree program in social work from a college or university approved by the Council of Social Work.
  • Oncology Social Work Certification (OSW-C) preferred.
  • Minimum of 3 years experience working with oncology patients in an acute care setting, outpatient setting, or support group setting.
