Clinical Social Worker

We are seeking a highly skilled and compassionate Licensed Clinical Social Worker to join our Virtual Care Center team at Reimagine Care. As a key member of our multidisciplinary team, you will provide psychosocial care to cancer patients and their families, working collaboratively with our clinical team to deliver patient-centered care.
Key Responsibilities:- Conduct comprehensive psychosocial assessments and develop treatment plans to address patients' social, emotional, and environmental needs.
- Provide individual counseling and support to patients and their families, using evidence-based therapies such as CBT.
- Facilitate patient and family support groups to foster a sense of community and connection.
- Develop and maintain relationships with community resources and organizations to provide patients with access to financial, spiritual, and emotional support.
- Collaborate with our clinical team to ensure seamless care coordination and communication.
- Active LCSW-C license and ability to obtain other state licenses as required.
- Master's degree in social work from a Council-approved program.
- Oncology Social Work Certification (OSW-C) and minimum 3 years of experience working with oncology patients.
- Excellent interpersonal and communication skills, with ability to work in a fast-paced, changing environment.
- Part-time opportunity with a maximum of 30 hours per week, with some evening and weekend hours required.
