Oncology Social Worker

Reimagine Care is seeking a highly skilled Licensed Clinical Social Worker to join our team. As a key member of our Virtual Care Center, you will provide psychosocial care to cancer patients and their families across the continuum of the cancer experience.
Key Responsibilities:
- Provide assessment, counseling, education, advocacy, and support programs to enhance patients' and families' ability to cope with physical, emotional, environmental, and social problems.
- Collaborate with the virtual care center and partner multidisciplinary teams to provide compassionate, patient-centered care.
- Develop and implement programmatic planning and standards of practice.
- Monitor selected clinical indicators and departmental audits.
Requirements:
- Active social work license at the Licensed Certified Social Worker - Clinical (LCSW-C) level.
- Graduate of a Master's degree program in social work from a college or university approved by the Council of Social Work.
- Oncology Social Work Certification (OSW-C).
- Minimum of 3 years experience working with oncology patients in either an acute care setting, outpatient setting, or support group setting.
Preferred:
- Spanish-speaking.
Work Schedule:
- Part-time team member working a max of 30 hours per week. Some evening and weekend hours may be required.
