About the Role

We are seeking a highly skilled and compassionate Social Worker I to join our team at the University of California, Irvine. As a key member of our case management team, you will play a vital role in ensuring the seamless transition of patients from hospital to home, providing essential support and resources to individuals and families in need.

Responsibilities
  • Conduct thorough assessments of patients' social and emotional needs, identifying areas for intervention and developing personalized plans to address these needs.
  • Collaborate with healthcare teams to ensure comprehensive care, including discharge planning, medication management, and follow-up appointments.
  • Provide emotional support and counseling to patients and their families, addressing concerns and fears related to illness, treatment, and recovery.
  • Connect patients with community resources, including social services, housing, and financial assistance, to promote stability and well-being.
  • Document patient interactions, progress, and outcomes, maintaining accurate and up-to-date records.
Requirements
  • Master's Degree in Social Work from an accredited university or Bachelor's Degree in Social Work from an accredited institution.
  • At least one year of experience in an acute medical facility or related setting, with a strong understanding of internal policies and State/Federal regulations.
  • Excellent written and verbal communication skills, with the ability to maintain effective working relationships across the health system.
  • Ability to work independently and as part of a team, prioritizing tasks and managing multiple responsibilities in a fast-paced environment.
Preferred Qualifications
  • Knowledge of University and medical center organizations, policies, procedures, and forms.
