Job Summary

The Nurse Practitioner will play a vital role in our medical team, providing advanced primary and specialty care to our patients. Key responsibilities include:

  • Conducting thorough physical exams and recording patient medical history
  • Creating personalized care plans and administering diagnostic tests
  • Collaborating with our multidisciplinary team to ensure seamless care coordination
Responsibilities

The successful candidate will be responsible for:

  • Examining and diagnosing patients, developing treatment plans, and prescribing medications as needed
  • Ensuring high-quality care by adhering to evidence-based guidelines and best practices
  • Collecting and maintaining accurate patient information, including medical history, allergies, and medications
  • Ordering and executing various tests, analyses, and diagnostic images to inform patient care
  • Analyzing reports and findings, diagnosing conditions, and developing treatment plans
  • Administering or prescribing treatments, including vaccinations and medications
  • Consulting with medical specialists as needed and referring patients to subspecialists within our network
Qualifications

To be considered for this role, candidates must possess:

  • A Master's Degree in a Nurse Practitioner Program (Family or Adult Geriatrics)
  • A current New York State License as a Nurse Practitioner
  • National Board Certification (AANP, ANCC, or ONCC)
  • Bilingual (Spanish-speaking) is a plus
  • EMR/EHR experience is a plus
