Care Manager RN Opportunity

CareFor is seeking a dedicated and compassionate Registered Nurse (RN) to join our Care Management team in a full-time capacity. As a Care Manager RN, you will be responsible for developing, implementing, and evaluating individualized care plans, acting as a patient advocate, and serving as a liaison between patients, their families, and other healthcare providers to meet the needs of individuals in the home and community setting.

Key Responsibilities:

* Conduct initial assessments of patients to determine needs, including medical, social, family, living arrangement, or any pertinent daily management, and reassess as needed.
* Develop, plan, implement, and evaluate ongoing care plans to meet individual needs.
* Make home visits according to patient needs and modify visit frequency as needed.
* Notify physicians of changes in condition and coordinate care with other healthcare or community providers to ensure patient needs are met.
* Accompany patients to physician appointments as needed to ensure optimal healthcare and advocate for patient needs.

Qualifications:

* Current RN licensure in Texas or in accordance with the Texas Board of Nursing rules for Nurse Licensure Compact (NLC).
* At least one year of experience as an RN in a clinical role.
* Effective written and verbal communication skills.
* Ability to read, write, and comprehend English.
* Basic computer skills and ability to navigate an Electronic Medical Record.
* Ability to triage patient needs and prioritize a daily schedule, knowing that needs change throughout the day.
* Critical thinking skills to solve problems creatively.
* Ability to lift, bend, squat, and present proof of a current driver's license and dependable insurance automobile.
* Neat appearance and at least 18 years of age.
