About the Job

We are seeking a skilled Registered Nurse to join our team in providing high-quality hospice care to patients in their homes. As a Hospice RN, you will be responsible for delivering compassionate and individualized care to patients and their families.

Responsibilities
  • Provide direct patient care in accordance with the plan of care and as ordered by the physician
  • Conduct thorough assessments of patients' physical, emotional, and spiritual needs
  • Develop and implement individualized care plans that address patients' needs and goals
  • Collaborate with the interdisciplinary team to ensure comprehensive care
  • Document patient information accurately and thoroughly
Requirements
  • Current RN licensure in the state of Oregon
  • At least 12 months of experience in hospice care within the past 3 years
  • Experience working in home health care settings
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
