Job SummaryWe are seeking a skilled Physical Therapist to join our CarePartners Home Health team. As a Primary Physical Therapist, you will manage an assigned caseload of patients, providing therapy and clinical case management to improve patient outcomes and manage resources effectively.
Key Responsibilities:- Conduct comprehensive assessments, examinations, and evaluations to develop and modify plans of care
- Provide treatment visits and discharge planning to facilitate patient engagement and achievement of goals
- Communicate with patients, caregivers, and physicians to ensure coordinated care
- Manage resources and ensure cost-effective, clinically effective services
- Bachelor of Science degree in Physical Therapy
- Current North Carolina license to practice Physical Therapy
- Driver's license required
- BLS/CPR Certification
- One to two years of related experience
