Job SummaryWe are seeking a skilled Physical Therapist to join our CarePartners Home Health team. As a Primary Physical Therapist, you will manage an assigned caseload of patients, providing therapy and clinical case management to improve patient outcomes and manage resources effectively.
Key Responsibilities:- Conduct comprehensive assessments and develop plans of care in conjunction with patients, caregivers, and physicians
- Provide treatment visits and discharge planning to facilitate patient engagement and achievement of goals
- Communicate with the care team to ensure coordinated services and positive patient experiences
- Bachelor of Science degree in Physical Therapy
- Current North Carolina license to practice Physical Therapy
- One to two years of related experience
