Physical Therapist Home Health Specialist

About the Role
CarePartners is seeking a dedicated Physical Therapist to join our Home Health team. As a Physical Therapist, you will play a vital role in providing high-quality patient care and improving outcomes for our patients.
Key Responsibilities
- Manage an assigned caseload of patients, providing therapy and clinical case management to improve patient outcomes and manage resources effectively.
- Conduct comprehensive assessments, develop and modify plans of care in conjunction with patients, caregivers, and physicians to achieve patient goals and clinical outcomes.
- Provide treatment visits and discharge planning to facilitate patient engagement and achievement of patient goals.
- Communicate handoff information and coordinate with Physical Therapist Assistants, other therapists, and care team members to support patient care throughout the home health continuum.
Requirements
- Current North Carolina license to practice Physical Therapy.
- Driver's license required.
- BLS/CPR Certification.
- One to two years of related experience.
- Preferred experience in clinical affiliations with a wide variety of acute care procedures, especially orthopedics, neurology, cardiopulmonary, and care of burns/open wounds.
