RN Clinical Manager

We are seeking a skilled RN Clinical Manager to oversee the day-to-day operations of our Advanced Illness Management (AIM) and Palliative Care department. The ideal candidate will have experience in healthcare administration and a passion for delivering high-quality patient care.
- Coordinate and manage all business affairs and clinical activities related to AIM and Palliative Care.
- Ensure clinical care delivery goals are met and exceeded.
- Manage a team of AIM Clinical Coordinators, AIM NPs, and AIM RNs.
- Oversee scheduling, frequency adherence, clinical coordination, medical record maintenance, and marketing functions.
- Manage incoming calls from partners and patients.
- Ensure Visit Freq compliance, scheduling PRN visits, post hospitalization follow-up with patient/family, and expediting discharge home with AIM communication with partners.
- Order labs, DME, and imaging as needed.
- Coordinate results with NPs, enter telephone encounters in partner EMR, and enter routine visit med reconciliation in partner EMR.
- Ensure transitions to HSP referrals to CTC for care type.
- Assist in identifying HSP eligible patients.
- Identify HH providers that can accept HH referrals from PCC.
- Identify and communicate with local partner office for lab drop-off.
Requirements
- Registered Nurse with Home Health, Hospice, and/or Palliative Care experience preferred.
- Bachelor's degree in nursing, Business Administration, or related field; or equivalent experience and knowledge in Health Care Operations required.
- Master's degree in business or related field preferred.
- Minimum of 2 years healthcare administration/operations management required.
- Must be able to work some nights/weekends on call.
- Certified in Hospice and Palliative Care - preferred.
