About the Role

We are seeking a highly skilled RN Clinical Manager to lead our Advanced Illness Management (AIM) and Palliative Care team. As a key member of our clinical leadership team, you will be responsible for coordinating and managing all day-to-day business affairs and clinical activities related to the administration of the AIM and Palliative Care department.

Key Responsibilities

* Ensure clinical care delivery goals are met and exceeded
* Manage a team of AIM Clinical Coordinators, AIM NPs, and AIM RNs
* Ensure scheduling, managing frequency adherence, clinical coordination, medical record maintenance function, and marketing functions are maintained for a team of patients and clinicians
* Manage incoming calls from partners and patients
* Manage Visit Freq compliance, scheduling PRN visits, post hospitalization follow up with patient/family-expediting discharge home with AIM communication with partners, schedule post hospitalization visits
* Order labs, DME, imaging
* Coordinate results with NPs, enter telephone encounters in partner EMR, and enter routine visit med reconciliation in partner EMR
* Ensure transitions to HSP referrals to CTC for care type
* Assist in appropriate identification of HSP eligible pts
* Identify HH providers that can accept HH referrals from PCC
* Identify and communicate with local partner office for lab drop off

Requirements

* Registered Nurse with Home Health, Hospice, and/or Palliative Care Experience preferred
* Bachelor's degree in nursing, Business Administration, or related field; or equivalent experience and knowledge in Health Care Operations required
* Master's degree in business or related field preferred
* Minimum of 2 years health care administration/operations management required
* Must be able to work some nights/weekends on call
* Certified in Hospice and Palliative Care - preferred
