Job Description

We are seeking a highly skilled and compassionate Registered Nurse to join our team as a Clinical Coordinator in Acute Care. As a key member of our interdisciplinary team, you will play a vital role in delivering high-quality patient care and ensuring the smooth operation of our dialysis services.

Responsibilities:

  • Provide direct patient care and coordinate with the healthcare team to ensure comprehensive care.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Collaborate with physicians, nurses, and other healthcare professionals to ensure seamless care transitions.
  • Conduct regular patient assessments and monitor vital signs to identify potential issues.
  • Communicate effectively with patients, families, and healthcare teams to ensure informed decision-making.

Requirements:

  • Current Registered Nurse (RN) license in the state of practice.
  • At least 18 months of registered nursing experience, preferably in acute care or dialysis.
  • Basic computer skills and proficiency in MS Word and Outlook.
  • Ability to work effectively in a fast-paced environment and prioritize tasks efficiently.
  • Strong communication and interpersonal skills, with a focus on patient-centered care.
