We are seeking a highly skilled and compassionate Registered Nurse to join our team as a Clinical Nurse Coordinator. As a key member of our healthcare team, you will be responsible for coordinating patient care and services, ensuring the highest level of quality and safety for our patients.
About the Role:- Coordinate patient care and services, including trend lab values, vital signs, and outcome management.
- Ensure patient safety, comfort, and well-being through effective communication and collaboration with the healthcare team.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Collaborate with the interdisciplinary team to ensure seamless care transitions and optimal patient outcomes.
- Current Registered Nurse (RN) license in the state of practice.
- At least 18 months of registered nursing experience.
- Dialysis experience is preferred.
- Certified Nephrology Nurse (CNN) or Certified Dialysis Nurse (CDN) is preferred.
