Job Summary

We are seeking a highly skilled Home Health Physical Therapist to join our team at Alternate Solutions Health Network LLC. As a key member of our interdisciplinary team, you will provide comprehensive skilled services to patients in their homes, promoting optimal recovery and independence.

Key Responsibilities
  • Develop and implement individualized plans of care to address patients' physical therapy needs, ensuring safe and effective treatment.
  • Conduct thorough assessments and evaluations to identify patients' strengths, weaknesses, and functional abilities.
  • Provide skilled physical therapy interventions, including exercises, modalities, and education, to promote patient recovery and independence.
  • Collaborate with patients, families, and healthcare providers to ensure seamless care coordination and communication.
  • Document patient progress and outcomes accurately and thoroughly, adhering to agency and regulatory requirements.
Requirements
  • Current licensure as a Physical Therapist in the state of employment.
  • Valid driver's license and auto insurance.
  • Ability to meet the physical demands of the job, including lifting, bending, and standing for extended periods.
