Job Summary:

The Home Health Therapist will be responsible for assessing patients' physical therapy needs, developing and implementing individualized treatment plans, and providing ongoing care and support to patients and their families. Key Responsibilities:
  • Assess Patients' Needs: Conduct thorough assessments of patients' physical therapy needs, including their medical history, functional abilities, and goals.
  • Develop Treatment Plans: Create individualized treatment plans that address patients' specific needs and goals, and provide ongoing care and support to patients and their families.
  • Provide Physical Therapy Services: Provide high-quality physical therapy services to patients in their homes, including exercises, stretches, and other interventions to promote independence and improve their overall health and well-being.
  • Communicate with Patients and Families: Communicate effectively with patients and their families to ensure that their needs are met and that they are informed and engaged in their care.
  • Collaborate with Healthcare Team: Collaborate with other healthcare professionals, including physicians, nurses, and social workers, to ensure that patients receive comprehensive and coordinated care.
  • Document Patient Progress: Document patients' progress and outcomes, including their response to treatment and any changes in their condition.
Requirements:
  • Physical Therapist License: Current physical therapist license in the state of employment.
  • Experience: Minimum of one year of experience as a physical therapist in an acute setting.
  • Home Care Experience: Desired experience in home care or a related setting.
  • Communication Skills: Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Knowledge and Skills: Knowledge of physical therapy best practices and the ability to apply them in a home care setting.
