Job Summary

We are seeking an experienced Senior Hospice and Palliative Care Nurse Practitioner to join our team at Hospice Savannah. This is a rewarding opportunity for a skilled healthcare professional to provide compassionate care to patients with terminal illnesses.

About the Role

The successful candidate will be responsible for diagnosing, treating, and managing patients who require palliative care services. As a primary care provider, you will work closely with the interdisciplinary team to develop individualized plans of care that address the physical, emotional, social, and spiritual needs of patients and their families.

You will have the autonomy to work in various settings, including hospitals, clinics, home care, and nursing homes, while consistently demonstrating excellence in patient care and adherence to our organization's values and standards.

Key Responsibilities
  • Conduct patient histories, physicals, and assessments to develop personalized plans of care
  • Collaborate with the interdisciplinary team to ensure seamless delivery of care
  • Work with patients and their families to establish realistic goals and expectations
  • Monitor and manage symptoms, providing support and education to patients and caregivers
  • Develop and implement strategies to improve quality of life and overall well-being
Requirements

To be considered for this role, you must have:

  • A Master's degree in Nursing with a specialization in Adult, Family, or Gerontology Nurse Practitioner
  • Board certification as an Adult, Family, or Gerontology Nurse Practitioner
  • Certification in Hospice and Palliative Care is highly desirable
  • At least three years of experience in hospice and palliative care, with a proven track record of delivering high-quality patient care
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with patients, families, and colleagues
Compensation and Benefits

The salary for this role is $120,000 - $150,000 per year, depending on experience. In addition to a competitive salary, we offer a comprehensive benefits package, including health insurance, retirement savings plan, paid time off, and continuing education opportunities.
