Job Summary:

Kaiser Permanente is seeking a skilled Licensed Practical Nurse (LPN) to join our team as a Clinical Business Health Worker (CBHW). As a CBHW, you will play a vital role in delivering high-quality patient care and supporting our clinicians in providing exceptional healthcare services.

Key Responsibilities:
  • Clinical Tasks: You will be responsible for preparing patients for office visits, performing focused nursing assessments, and utilizing standardized tools to identify health maintenance gaps. You will also assist with clinical procedures, such as blood draws, and provide health education to patients.
  • Clinical Procedures: As a CBHW, you will perform a variety of clinical tasks, including diagnostic tests, therapies, and procedures. You will work under the direction of a clinician and follow established protocols and policies.
  • Practice Management Activities: You will assist with scheduling patient appointments, responding to messages, and facilitating follow-up duties on patient plans of care. You will also help coordinate patient care for patients with complex health needs.
Requirements:
  • Current LPN licensure in the state of California
  • Ability to work in a fast-paced environment and prioritize tasks effectively
  • Strong communication and interpersonal skills
  • Ability to work collaboratively with clinicians and other healthcare team members
