Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team as a Community-Based Health Worker (CBHW). As a CBHW, you will play a vital role in delivering high-quality patient care and contributing to the success of our healthcare team.

Key Responsibilities
  • Provide culturally responsive care to patients, incorporating an awareness of potential barriers to accessing services and care that may result in poor health outcomes.
  • Implement care delivery and social support models aimed at achieving health equity for the Black community.
  • Assist with clinical tasks, such as preparing patients for office visits, performing focused nursing assessments, and implementing planned nursing and medical interventions.
  • Contribute to the development, implementation, or modification of individualized patient plans of care under the direction of a registered nurse (RN) or clinician.
  • Provide health education and advice on routine or self-limiting medical conditions, procedures, tests, surgeries, self-injections, etc.
  • Utilize standardized tools to determine patients' level of engagement in self-care and document all pertinent data and outcomes of nursing care according to established procedures.
  • Support blood draws (phlebotomy) and perform diagnostic tests, therapies, and procedures within the scope of practice, maintaining proficiency according to department standards.
  • Participate in population-based care outreach activities, including health maintenance screening, as directed by a clinician.
  • Collaborate with the healthcare team to provide patient-centered care, addressing the physical, emotional, and social needs of patients.
Requirements
  • Graduate of an accredited school of nursing.
  • Practical Nurse License (Washington) within 6 months of hire or Compact License: Practical Nurse within 6 months of hire.
  • Practical Nurse License (Oregon) within 6 months of hire.
  • Basic Life Support required at hire from the American Heart Association.
  • Demonstrated clinical nursing skills, interpersonal and customer service skills, communication skills, and literacy in using computers and keyboarding.
  • Ability to work independently and efficiently under pressure, with flexibility and ability to reprioritize workflow to handle urgent/emergent demands of the team effectively.
