Job Summary

We are seeking a highly skilled and compassionate Case Manager to join our team at Houston Methodist. As a Case Manager, you will play a critical role in ensuring the seamless transition of patients from hospital to home, while also promoting high-quality, patient-centered care.

Key Responsibilities
  • Collaborate with healthcare teams to develop and implement comprehensive care plans that meet the unique needs of each patient.
  • Conduct thorough assessments of patients' medical, social, and financial situations to identify potential barriers to care.
  • Develop and implement strategies to address these barriers, ensuring timely and effective transitions of care.
  • Communicate regularly with patients, families, and healthcare providers to ensure that all parties are informed and engaged in the care planning process.
  • Monitor and evaluate the effectiveness of care plans, making adjustments as needed to ensure optimal outcomes.
Requirements
  • Bachelor's degree in nursing or related field.
  • Minimum of 5 years of experience in case management or a related field.
  • Strong knowledge of healthcare systems, policies, and regulations.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
