Children's Habilitation Center is accredited by the Illinois Department of Public Health for 67 beds, dedicated to serving infants and children with a commitment to our core values: compassion, innovation, and holistic care.

Over the years, our services have evolved, leading to significant changes within our facility. The increased demand for care has necessitated the recruitment of additional professional and licensed personnel.

Our expanded team now includes Respiratory Therapists, Trach Wound Nurses, Certified Nursing Assistants (CNAs), Physical Therapists, Certified Occupational Therapist Assistants, a Registered Dietitian, and a Psychologist.


REQUIRED QUALIFICATIONS:
Certified Therapeutic Recreation Specialist (CTRS), Occupational Therapist Registered and Licensed (OTR/L), or a Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW).

If this resonates with your values and aspirations for a company culture, consider joining our team. As an Activities Coordinator at Children's Habilitation Center, you will play a vital role in enhancing the lives of children by:

  • Implementing activities and after-school programs in accordance with the established schedule.
  • Documenting progress and attendance for all activities, both group and individual.
  • Completing daily documentation to ensure exceptional care for all children.
  • Assisting in the development and execution of habilitation plans.
  • Maintaining records and preparing requested reports.
  • Submitting activity notes by the 10th of each month.
  • Participating in the ongoing interdisciplinary evaluation of individual residents' needs.
  • Ensuring written permission from the resident's physician is obtained for participation in the activity program.
  • Coordinating activities with other departments to ensure clear communication and alignment of goals.
  • Assisting in the transportation of students to and from activities.
  • Contributing to the planning and execution of special events.
  • Coordinating holiday visits from various groups and foundations.
  • Proposing ideas for in-house and community activities.
  • Attending seminars, workshops, and facility or departmental meetings as necessary.
