Hospice Registered Nurse Case Manager

We are seeking a skilled Hospice Registered Nurse to provide exceptional end-of-life care to patients and their families in Mandeville, Covington, Slidell, and Hammond, Louisiana.

About the Role

As a Hospice Registered Nurse Case Manager, you will offer support to patients and their families with empathy and professionalism, ensuring comfort, dignity, and quality of life. Your responsibilities will include managing pain and symptoms, coordinating with interdisciplinary teams, and delivering holistic care tailored to individual needs.

Key Responsibilities
  • Provide home-based care to patients with six months or less to live, focused on quality of life
  • Work as part of an interdisciplinary team to carry out the patient's plan of care
  • Perform patient assessments, check vitals, evaluate breathing, monitor pain, administer prescribed medication and treatments
  • Ensure accessibility of medical equipment, prescribed oxygen, medication, and other supplies
  • Educate family on the patient's prognosis, changes in care plan, medication, and hospice goals
  • Take a holistic approach to care that engages the patient's mind, body, spirit, and family
Requirements
  • Graduate of an accredited nursing program and active RN license in Louisiana
  • Minimum of 1 year of hospice nursing experience
  • CPR certification
  • Valid Louisiana driver's license, auto insurance, and reliable transportation
  • Knowledge of state and federal home health regulations
  • Clinical proficiency in nursing specialty, strong communication skills, and compassion
