Job Title: Outreach Specialist

Montefiore Medical Center is seeking a highly motivated and experienced Outreach Specialist to join our team. As a key member of our outreach team, you will play a critical role in providing evidence-based HIV prevention, risk reduction education, and capacity building to individuals and grassroots/community-based organization (GR-CBO) partners.

Responsibilities:
  • Provide HIV prevention, risk reduction education, and capacity building to GR-CBO partners to improve their capacity to implement social networking and other proven engagement strategies for HIV self-test kit distribution and PrEP education/uptake.
  • Support the provision of referrals for essential and supportive services, ensure linkage to care, and support recruitment for MPP's Community Engagement Group, intervention activities, and condom distribution.
  • Conduct eligibility screenings, intake, and data entry for services provided, aimed at improving the health of MSM, LGBTQ+, and high-risk individuals in the Bronx and NYC.
Requirements:
  • Bachelor's Degree
  • At least 1 year of experience in healthcare and/or care coordination and care management
  • Knowledge of care coordination and local community resources
