Children's Aid is seeking a dedicated and compassionate Case Planner to join our team. As a key member of our Foster Care program, you will work closely with children, families, and our multidisciplinary team to provide intensive casework services and fieldwork support. Your primary goal will be to ensure the well-being and safety of our youth, while promoting permanency and stability in their lives.
Key Responsibilities:
Provide casework services to children and families, including assessment, planning, and implementation of interventions.
Collaborate with our team of therapists, education specialists, and social workers to develop and implement individualized plans for each child.
Conduct outreach and connect children and families with agency and community resources to support their needs.
Utilize our child welfare computer system, CONNECTIONS, to track and manage case information.
Coordinate Family Team Conferences and participate in case consultations to ensure comprehensive support for our youth.
Requirements:
Bachelor's degree in Social Work, Psychology, or a related field (Master's degree preferred).
Experience in foster care or a related field (bilingual skills a plus).
Valid driver's license and ability to work a flexible schedule.
