POSITION SUMMARY
The Family Support Case Manager is responsible for delivering essential services to diverse populations, including refugees and asylees. This role focuses on identifying and addressing barriers to client safety and well-being, while facilitating stabilization through self-sufficiency and long-term integration strategies for clients and their families. The Family Support Case Manager plays a critical role in ensuring that both basic and specialized needs of clients are met, particularly for those from vulnerable backgrounds.
KEY RESPONSIBILITIES
- Offer client-centered counseling and crisis intervention, including assistance with benefit enrollment, health and mental health services, employment support, and language training.
- Develop and implement service plans, ensuring timely case reviews and closures in accordance with compliance standards.
- Provide ongoing education on resettlement topics, including medical benefits, public health, housing, and workplace etiquette.
- Connect clients with trained mentors to enhance their support network.
- Assist clients in navigating and accessing external services and public benefits.
- Maintain and update client databases with accurate information, assessments, and referrals.
- Engage in program network meetings and collaborate with community partners to enhance resource availability for clients.
- Participate in training and conferences relevant to program objectives.
- Ensure comprehensive documentation in client case files throughout the service period.
- Compile and submit necessary data for reports in a timely and accurate manner.
- Work collaboratively with colleagues to promote equitable service delivery and proactive client support.
- Adhere to all federal, state, and agency regulations.
- Perform additional duties as assigned.
QUALIFICATIONS
To excel in this role, candidates must demonstrate the ability to fulfill essential duties effectively. The following qualifications are necessary:
- Knowledge of refugee resettlement practices and principles is preferred.
- Understanding of U.S. immigration laws and regulations is advantageous.
- Familiarity with local community resources and access methods.
- Cross-cultural sensitivity and a commitment to working with diverse populations.
- Strong organizational, problem-solving, negotiation, and communication skills, including proficiency in Microsoft Office applications.
- Ability to communicate effectively with clients who may have limited English proficiency.
- Reliable transportation and a valid driver's license are required.
- Commitment to the mission and values of St Johns Lutheran Ministries Inc.
EDUCATION AND EXPERIENCE
Associates degree or two years of experience in social work, human services, or a related field is preferred. Equivalent combinations of education and experience will be considered. Fluency in a second language is a plus.
