About the Role:
The Care Manager PRN at CHRISTUS Health plays a vital role in ensuring seamless patient care progression through the continuum of care. As a key member of the multidisciplinary team, you will work collaboratively with patients, families, physicians, and other healthcare professionals to develop and implement effective care plans.
Key Responsibilities:
- Conduct thorough assessments of patients' social, emotional, and financial factors to identify potential barriers to care.
- Develop and monitor patient care plans to ensure optimal outcomes and efficient use of resources.
- Coordinate and facilitate patient care progression throughout the continuum of care, ensuring timely and effective transitions.
- Act as a patient advocate, providing support and guidance on treatment decisions and end-of-life issues.
- Collaborate with interdisciplinary teams to identify and resolve barriers to care, ensuring patient safety and satisfaction.
Requirements:
- Certificate, Associate, or Bachelor's degree in nursing or a related field.
- Experience in clinical or acute care settings preferred.
- LVN/LPN, RN, LBSW, LMSW, or LCSW licensure in the state of employment required.
Work Schedule: PRN, Per Diem As Needed
