Job Summary:
The Care Manager at CHRISTUS Spohn Hospital Corpus Christi-South plays a vital role in ensuring seamless patient care progression through the continuum of care. This dynamic professional collaborates with patients, families, physicians, and multidisciplinary team members to develop and implement individualized plans of care, promoting continuity, smooth transitions, patient satisfaction, and safe care delivery.
Key Responsibilities:
- Conduct thorough assessments of patient needs, identifying potential barriers to effective care and developing strategies to overcome them.
- Collaborate with healthcare teams to coordinate patient care, ensuring timely and efficient transitions between care settings.
- Develop and monitor patient plans of care, ensuring alignment with departmental and organizational goals, objectives, and standards of performance.
- Provide education and support to patients and families, empowering them to make informed decisions about their care.
- Facilitate communication and coordination between healthcare providers, payors, and community resources to ensure seamless care delivery.
- Identify and escalate local and system barriers impeding diagnostic or treatment progress, ensuring timely resolution and minimizing delays.
- Work closely with medical staff, nursing staff, and ancillary staff to eliminate barriers to efficient care delivery, promoting a culture of collaboration and excellence.
- Assess patient needs for discharge planning and continuing care/resource support, making recommendations for post-acute levels of care and community resources.
- Ensure accurate and timely documentation of patient care activities, maintaining regulatory compliance and departmental standards.
Requirements:
- Graduate of an accredited school of nursing or Bachelor's Degree in Social Work (BSW) required.
- Experience in the clinical or acute care setting preferred.
- LVN/LPN or LBSW licensure in the state of employment required.
