The Care Manager II is a key member of the healthcare team, responsible for ensuring seamless patient transitions throughout the continuum of care. This role requires strong clinical expertise, excellent communication skills, and the ability to navigate complex healthcare systems.
Key Responsibilities- Collaborate with patients, families, and healthcare providers to develop and implement individualized care plans
- Coordinate patient care transitions, including hospital-to-home, hospital-to-rehabilitation, and hospital-to-nursing home
- Identify and address barriers to care, including social determinants of health and access to community resources
- Provide education and support to patients and families on health management and self-care
- Work closely with interdisciplinary teams to ensure timely and effective care delivery
- Graduate of an accredited school of nursing (BSN preferred) or Masters Degree in Social Work (MSW) required
- Two or more years of clinical experience, with one year in the acute care setting preferred
- RN or LMSW licensure in the state of employment required
- Certification in Case Management preferred
The Care Manager II will work in a fast-paced, dynamic environment, collaborating with multidisciplinary teams to deliver high-quality patient care. This role requires strong critical thinking and problem-solving skills, as well as the ability to work independently and as part of a team.
