Job Title: Case Manager

We are seeking a skilled Case Manager to join our team at Susquehanna University. As a Case Manager, you will be responsible for providing comprehensive case management services to an assigned caseload, including conducting intake processes, psychosocial assessments, and documentation of client progression.

Key Responsibilities:
  • Conduct intake processes with new clients within 24 hours of admission
  • Conduct psychosocial, vocational/educational, and housing assessments
  • Provide accurate, complete, and timely documentation of client progression
  • Prepare housing applications and assist clients with the housing application/interview process
  • Monitor and assess client progression/regression in behavior and/or appearance
  • Facilitate group and individual counseling, workshops, and recreational activities
  • Conduct room/dorm visits/inspections to assess client personal hygiene and room cleanliness
  • Provide comprehensive case management services to an assigned caseload
  • Develop initial service plans and conduct service plan reviews as needed
  • Provide referrals for in-house and community-based medical, mental health, and supportive social services
Requirements:
  • Bachelor's Degree in Human Services field preferred
  • Two years' experience with homeless population
  • Ability to work with a culturally diverse population
  • Experience in residential setting, group work, tenant organization, case management, referral, and/or community outreach strongly preferred
  • Bilingual Spanish speaking strongly preferred
