Job Overview:

The Senior Community Care Specialist plays a pivotal role in a field unit, delivering essential services directly to individuals in Long Term Care Services.

This position involves conducting home assessments, engaging with older adults or individuals with disabilities, documenting vital information, evaluating service needs, and formulating personalized service plans. Responsibilities also include verifying information, clarifying program benefits and requirements, referring clients to suitable services, and ensuring that authorizations are accurately recorded in automated systems.

The specialist is responsible for authorizing and overseeing Community Care Services Eligibility (CCSE) to ensure that services align with the needs of individuals.

Additional responsibilities may encompass acting as a supervisor in the absence of the unit leader, reviewing cases, providing guidance and policy clarification to colleagues, training new staff members, or preparing specialized reports. Proficiency in computer operations is required, and involvement in special projects for the region may also be expected.

Key Responsibilities:

  • Assess eligibility for services in accordance with established program policies and procedures.
  • Design and implement tailored service plans for clients.
  • Provide continuous case management to monitor client conditions and evaluate the effectiveness of services in addressing various issues.
  • Maintain comprehensive case records to ensure a documented history for each individual.
  • Prepare detailed reports and records to furnish management with information regarding program operations.
  • Assist the supervisor with administrative tasks, which may include stepping in as acting supervisor, organizing staff meetings, delegating work, offering counseling, and overseeing staff performance and caseloads.
  • Utilize computer systems to document, update, and access case records.
  • Interpret policies for team members.
  • Mentor and support staff development.
  • Assist in the onboarding and training of new personnel.
  • Support the unit supervisor in generating reports or addressing complex case issues.
  • Act as a resource for challenging or intricate cases.
  • May review cases for the unit.
  • May engage in recruiting and supervising volunteers.
  • Perform related duties as assigned.
  • Travel up to 65% of the time for home visits, training sessions, and meetings.
Required Knowledge, Skills, and Abilities:

  • Understanding of gerontology, the aging process, and the unique challenges faced by individuals in Long Term Care Services.
  • Familiarity with interviewing techniques to gather personal information, make inquiries, and resolve discrepancies.
  • Strong oral and written communication skills.
  • Proficient knowledge of the English language, including vocabulary, grammar, and effective composition for correspondence and case reporting.
  • Ability to actively listen to clients, empathize with their feelings, and present relevant options regarding fees, bills, and potential outcomes.
  • Capability to complete tasks within designated timeframes.
  • Skill in assessing living conditions, including both physical and emotional environments.
  • Ability to prioritize tasks based on importance and deadlines, set goals, and manage time effectively.
  • Capacity to monitor unit operations and observe employee performance.
  • Ability to build rapport with clients, colleagues, contract agency staff, and representatives from other governmental agencies.
  • Skill in communicating with community members to solicit assistance for clients, foster support, and coordinate resources and programs.
  • Ability to read and analyze cases accurately.
  • Skill in conveying policies and procedures to team members.
  • Proficiency in operating computers and general office equipment.
  • Knowledge of resources available for individuals who are older or have disabilities.
