HealthFlex is seeking a highly organized and detail-oriented Medical Records Clerk to manage and maintain patient records for our home health agency. This role is responsible for ensuring the accuracy, confidentiality, and security of medical records in compliance with healthcare regulations and agency policies.

Key Responsibilities
  • Record Management: Organize, update, and maintain accurate electronic medical records, including uploading physical orders, Health Management Profile (HMP), lab results, and other patient care documents.
  • Orders Management: Responsible for distribution and timely collection of signed physician orders via DocuSign, fax, Forcura, courier, etc.
  • Data Entry: Accurately input patient information into the agency's electronic health record (EHR) system.
  • Compliance: Ensure all records comply with state, federal, and agency regulations, coordinating collection of any missing information or documentation.
  • Confidentiality: Ensure patient confidentiality is met by adhering to Health Insurance Portability and Accountability Act (HIPAA) and Protected Health Information (PHI) rules and regulations.
  • Audit Preparation: Assist with preparing records for audits and reviews.
  • Coordination: Collaborate with healthcare providers, patients, and administrative staff to facilitate the flow of information, including coordination of Durable Medical Equipment (DME) and working with Case Managers regarding resumptions of care post hospitalization discharge.
  • Communication: Handle difficult and/or sensitive matters with careful judgment and tact on a consistent basis, escalating to leadership as appropriate.
  • Training: Provide training to new staff as well as cross-train in other areas for coverage.
  • Administrative Duties: Perform tasks such as phone coverage, print/scan/faxing of documents, preparing mail/shipments, distributing documents, and courier coordination.

Requirements
  • Previous experience in medical records management, preferably in home health or similar setting.
  • Strong organizational skills, attention to detail, proficiency in EHR systems, understanding of HIPAA regulations.
  • Excellent written and verbal communication skills.
