Job Summary

The Enhanced Care Management Supervisor is a key role within our organization, responsible for providing guidance and support to the Care Management team. This includes assisting with the management of health center questions and conflicts, as well as monitoring staff productivity and performance.

Key Responsibilities
  • Provide comprehensive care management and care coordination for patients with complex health problems and psychosocial issues.
  • Assist with the management of health center questions and conflicts.
  • Monitor staff productivity and performance.
  • Provide training and guidance to the Care Management team.
  • Collaborate with other departments to ensure seamless care coordination.
Requirements
  • Associate degree or equivalent combination of education and relevant care coordination experience.
  • Minimum of 3 years of care coordination experience in an outpatient clinical setting, with at least 1 year of enhanced care management experience.
  • 1 year of work experience in a lead capacity.
  • Experience in chronic disease management, case management, utilization management, and acute care or Certificate of Completion in Care Management.
  • Knowledge of medical terminology and care coordination.
  • Knowledge of the HIPAA Privacy and Security Rule.
  • Bilingual in English and Spanish.
  • Computer proficiency, including Electronic Health Records and Microsoft Office Suite.
  • Reliable transportation for travel to trainings and meetings, as needed.
Desired Qualifications
  • Bachelor's Degree in social work, human services, counseling, human relations, or related field and ECM experience.
