Job Overview

Position Title: Peer Support Specialist
Compensation: $22.12 per Hour


Objective:

The Peer Support Specialist (PSS) engages with individuals in both individual and group environments to assist them in achieving their fullest potential. By exemplifying personal empowerment, the PSS instills hope and motivation, aiding clients in realizing their aspirations and implementing necessary lifestyle adjustments for improved overall health outcomes.

Role Summary:

Under the guidance of program management, the PSS aids clients in articulating their recovery objectives, tracks their progress, demonstrates coping strategies and self-help techniques derived from their own recovery journey, and advocates for clients in accessing pertinent services.

Key Responsibilities:

  • Establish professional relationships grounded in mutual learning and the core principles of hope, equality, respect, personal accountability, and self-determination to support clients, free from traditional expert/patient dynamics.
  • Facilitate or co-facilitate health and wellness-oriented client groups.
  • Inspire hope through personal experiences and utilize life stories intentionally to assist others.
  • Encourage recovery, wellness, and healthy lifestyle choices.
  • Promote active involvement in managing health and assist clients with behavioral health challenges in identifying and pursuing desired changes in wellness.
  • Implement peer-led, evidence-based practices (EBP) that empower clients to set goals, enhance healthy habits, optimize their time with healthcare professionals, diminish self-stigma, and reclaim lost roles and ambitions in life.
  • Acquire and maintain relevant EBP training to deliver these services effectively.
  • Support clients in overcoming social or emotional obstacles to remain motivated towards their goals.
  • Share strategies for managing health conditions.
  • Demonstrate profound respect and empathy for the effects of serious behavioral health challenges on all life aspects, providing understanding through shared experiences of emotional and psychological distress.
  • Bridge the gap between clients and healthcare professionals, assisting them in seeking clinical and community resources as needed.
  • Identify resources for clients, such as access to healthy food options, convenient exercise locations, or low-cost/free social activities.
  • Assist clients in completing necessary paperwork.
  • In specific situations, provide transportation for urgent appointments when barriers hinder attendance.
  • Collaborate with Care Coordinators and Health & Wellness Coaches to keep clients engaged by following up and monitoring their progress between visits.
  • Conduct regular maintenance calls for a defined period following the conclusion of treatment.
  • Help create a personalized, client-centered service plan.
  • Document service provision in accordance with clinical and administrative protocols.
  • Ensure appropriate Release of Information (ROI) forms are completed.
  • Conduct brief assessments to track client progress as necessary.
  • Adhere to HIPAA and other Federal or State Data Privacy regulations regarding organizational data handling.
  • Utilize technology to complete and monitor job-related tasks.
  • Meet established personal and departmental objectives and activity metrics.
  • Exemplify and promote the mission, vision, and values of Mental Health Partners through behavior and job performance, while adhering to policies, procedures, and standards of conduct.
  • Engage in communication with other staff, clients, customers, and partners.
  • Maintain regular attendance to fulfill essential job functions.
  • Perform additional tasks as assigned.

Specialized Responsibilities:

Integrated Health Home PSS – This role is part of a collaborative initiative to provide comprehensive, whole-person care aimed at enhancing the overall health of our clients.

  • Meet with clients during same-day primary care visits to welcome them and briefly assess their needs.
  • Demonstrate commitment to the goals of the Integrated Health Home and a recovery-oriented approach.
  • Work in a diverse environment to effectively communicate client needs and team actions required for support.
  • Participate in quality improvement initiatives.

Qualifications:

  • High school diploma or GED is required.
  • Must obtain Colorado Peer and Family Specialist Certification (CPFS) within 9 months of hire and maintain certification during employment.
  • Must have lived experience with behavioral health issues or substance use disorders, preferably within the community mental health system.
  • Ability to learn and maintain knowledge of community resources available to clients.
  • If driving clients, must be at least 21 years of age and possess a valid CO Driver's License with a good driving record.
