**Job Summary:**

We are seeking a highly skilled and compassionate Nurse Practitioner to join our team at Helping You Heal Llc. As a key member of our interdisciplinary team, you will be responsible for providing high-quality care to individuals with serious mental illness.

About the Role:

The successful candidate will work closely with our multidisciplinary team to provide comprehensive care, including assessment, treatment, and support services. This will involve conducting assessments, developing individualized treatment plans, and collaborating with community physicians to ensure seamless care coordination.

Responsibilities:

  • Conduct thorough assessments of patients' physical and mental health needs
  • Develop and implement individualized treatment plans in collaboration with the interdisciplinary team
  • Provide medication management services, including dispensing and administering medications as needed
  • Monitor patients for side effects and adverse reactions, and develop strategies to maximize medication adherence
  • Collaborate with the team psychiatrist to coordinate medical assessments and referrals to community physicians
  • Educate patients and their families on mental health conditions, treatment options, and self-management techniques
  • Promote health education and prevention activities to support patients' overall well-being
  • Cultivate strong relationships with community providers to ensure effective care coordination

Requirements:

  • Degree in Nursing (RN, LPN, or Nurse Practitioner)
  • Minimum 1 year of experience working with adults with serious mental illness
  • Working knowledge of psychiatric medications and treatments

A competitive salary of $65,000 - $80,000 per year, depending on experience, and a range of benefits, including health insurance, retirement plan, and paid time off.
