Children's National Hospital is seeking a skilled Licensed Practical Nurse to join our Home Care team. As a LPN, you will provide high-quality nursing care to patients in their homes, working closely with our multidisciplinary team to ensure seamless care coordination.
Job Summary:We are looking for a compassionate and detail-oriented LPN to provide nursing care to patients in their homes. The ideal candidate will have a strong background in pediatric nursing and experience working in home care settings. As a member of our team, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.
Responsibilities:- Provide high-quality nursing care to patients in their homes, including administering medications, wound care, and vital sign monitoring.
- Conduct thorough assessments of patients' physical and emotional needs, developing and implementing individualized care plans to meet those needs.
- Collaborate with our multidisciplinary team to ensure seamless care coordination, including communication with physicians, therapists, and other healthcare professionals.
- Document patient care and progress in a timely and accurate manner, maintaining confidentiality and adhering to HIPAA guidelines.
- Participate in ongoing education and training to stay current on best practices and industry developments.
- Current licensure as a Licensed Practical Nurse in the District of Columbia or Maryland.
- At least one year of experience in pediatric nursing, preferably in a home care setting.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
- Ability to lift up to 50 pounds and stand for long periods, with frequent bending and stooping.
- Valid driver's license and reliable transportation.
