As a key member of our team, you will be responsible for providing individualized support and guidance to clients in a homelike environment, promoting their independence and self-determination.
Key Responsibilities- Supervise and support clients in daily living activities, including meal preparation, personal hygiene, and medication management.
- Develop and implement individualized service plans to meet clients' unique needs and goals.
- Provide counseling and guidance to clients to promote healthy social interaction and emotional development.
- Ensure the safety and security of the facility and its residents.
- Maintain accurate and timely documentation of client progress and service delivery.
- Collaborate with other team members to ensure continuity of care and services.
- Associate's degree in human services or a related field.
- High school diploma or equivalent plus 6 months to 1 year of related mental health experience.
- Valid PA driver's license required.
