Job Summary:
We are seeking a highly skilled and experienced Nurse Manager to lead our Hospital at Home (HaH) program. As a key member of our team, you will be responsible for providing exceptional patient care, leading interdisciplinary teams, and ensuring compliance with regulatory requirements.
Key Responsibilities:
- Manage the day-to-day operations of the HaH program, including patient identification, coordination with the Emergency Department team, and scheduling of associates and interdisciplinary teams.
- Conduct disciplinary conferences, document individual counseling and disciplinary actions, and perform managerial responsibilities related to recruitment, hiring, coaching, development, and evaluation of staff.
- Communicate policies and procedures to ensure compliance with Health First and governing expectations, and facilitate high-quality, cost-effective outcomes focused on whole-patient orientation and self-management decision support.
- Provide appropriate delegation and supervision of patient care activities, collaborate with the hospital's Quality department to monitor patient outcomes, and promote quality patient care and customer service/satisfaction.
- Ensure compliance with The Joint Commission and other organizational and regulatory agency standards, and maintain accurate and timely records for all patient care.
Requirements:
- Bachelor's degree in Nursing
- Two years of acute care experience and one year of managerial experience in a related field
- Current Florida RN license
- American Heart Association BLS Healthcare Provider certification
- Eligible Level II background screening
Preferred Requirements:
- Master's degree in Nursing
- Two years of managerial experience leading interdisciplinary teams and interfacing with hospitalists and other hospital functions
Physical Demands:
- Travel to/from various Health First locations and patient residences
- Visual acuity and hand-eye coordination to perform clinical tasks
- Able to lift, push, or move equipment weighing up to 25 pounds with or without assistance
- Stand, walk, bend, and stoop for 90% of assigned shift
