**Job Summary:**

We are seeking a highly skilled Universal Care Specialist to join our team at the Agency on Aging of South Central Connecticut. This is an exciting opportunity to make a meaningful difference in the lives of older adults and individuals with disabilities.

**Key Responsibilities:**

  • Develop and implement individualized care plans that meet the unique needs of our participants.
  • Coordinate and oversee the delivery of services, ensuring quality and consistency.
  • Build strong relationships with participants, families, and caregivers to ensure seamless support.
  • Maintain accurate records of services and interactions.
  • Participate in ongoing educational and professional development opportunities to stay current in care management practices.

**Requirements:**

  • Bachelor's degree in Social Work, Nursing, or a related field.
  • A minimum of 2 years experience in care management or a similar role.
  • Excellent communication, organizational, and problem-solving skills.
  • Self-motivation and ability to work independently and as part of a team.
  • Proficiency with electronic health records and relevant technology.
