Job Description
As a Universal Care Manager with the Agency on Aging of South Central Connecticut, you will have the opportunity to make a meaningful difference in the lives of those we serve. Your role will involve organizing and overseeing personalized services, guaranteeing that our clients enjoy a life of independence and dignity within the comfort of their communities.

Responsibilities
  • Assess the needs and preferences of our participants to develop effective care plans.
  • Collaborate with participants, their families, and other caregivers to implement tailored care plans.
  • Ensure alignment between services provided and the goals and needs of our clients.
  • Provide continuous support and advocacy for our participants, addressing concerns proactively.
  • Maintain accurate and up-to-date records of all services and interactions.
  • Promote partnerships with community resources and service providers to strengthen our support network.
  • Stay at the forefront of care management practices through ongoing education and professional development.

Requirements
This role requires a strong candidate who possesses:
  • A Bachelor's degree in Social Work, Nursing, or a related discipline.
  • A minimum of 2 years experience in care management or a similar role.
  • An understanding of the unique needs and challenges facing older adults and individuals with disabilities.
  • Excellent communication, organizational, and problem-solving skills.
  • Self-motivation and ability to work independently and as part of a team.
  • Competency with electronic health records and relevant technological tools.

Benefits and Salary
