Position Overview: RN Care Manager

Role Summary:

The RN Care Manager will play a crucial role in supporting healthcare operations, focusing on patients with chronic conditions such as CHF and COPD. This position is integral to enhancing patient education, addressing social determinants of health, and ensuring seamless service coordination across various healthcare settings.

Key Responsibilities:

  • Provide comprehensive disease education to patients and their families.
  • Identify and address social factors impacting health outcomes.
  • Collaborate with healthcare providers to ensure a cohesive care approach.
  • Coordinate necessary services to optimize patient care.

Qualifications:

A Bachelor of Science in Nursing (BSN) is required, along with a valid New York State Registered Professional Nurse license. Candidates with an Associate Degree in Nursing (AD) may be considered if they meet established performance standards. Previous experience in care management within settings such as disease management, home health, or hospital environments is essential. Certification in Case Management (CCM) is preferred.

Experience Requirements:

  • A minimum of 5 years of nursing experience in care management is required.
  • Proficiency in electronic medical records (EMR) is necessary.
  • Demonstrated leadership experience in project management is essential.
  • Strong knowledge of community resources and insurance plans is required.

Skills and Competencies:

Exceptional oral and written communication skills are vital. Candidates should exhibit excellent customer service abilities and the capacity to work autonomously while prioritizing care coordination effectively. Proven leadership capabilities and the ability to thrive in a collaborative team environment are essential. The ideal candidate will be highly organized, detail-oriented, and capable of managing multiple tasks efficiently.

Understanding and applying effective behavioral and educational strategies, including motivational interviewing and self-management support, is crucial. Recognizing the influence of cultural, social, economic, and behavioral factors on health service delivery and responding to diverse patient needs is a key aspect of this role.

Compensation:

The pay range for this position is $44.95 per hour, commensurate with experience, skills, and educational background. Compensation for exempt positions under the Fair Labor Standards Act (FLSA) will align with the stated hourly rates, which may vary based on location.
