Position Overview: RN Care Manager
Support for Various Locations
This role involves serving as an ambulatory care registered nurse dedicated to managing patients with multiple chronic conditions, including Congestive Heart Failure (CHF) and Chronic Obstructive Pulmonary Disease (COPD). The RN Care Manager will prioritize patient education regarding their diseases, assess and address social determinants of health, and facilitate necessary services while collaborating with healthcare providers to ensure seamless care throughout the healthcare system, ultimately enhancing patient health outcomes.
Qualifications Required:
A Bachelor of Science in Nursing (BSN) is mandatory, along with a valid New York State Registered Professional Nurse license. Candidates with an Associate Degree in Nursing (AD) may be considered if they meet performance criteria. Previous experience in care management is essential, with a background in disease management, home health care, hospital, or physician office settings being preferred. Certification in Case Management (CCM) is advantageous.
Experience and Skills:
A minimum of 5 years of RN experience in Care Management is required. Familiarity with electronic medical records (EMR) is essential, along with previous experience in project leadership. Candidates should possess a strong understanding of community resources and general knowledge of insurance plans and clinical resource management.

Compensation:
The pay range for this position is $44.95 per hour, with compensation determined by experience, skills, and educational background. Salary for exempt positions under the Fair Labor Standards Act (FLSA) will align with the stated hourly rates, which may vary based on location.
