Alternate Solutions Health Network LLC is seeking a skilled Home Health Occupational Therapist to join our team in Akron, Ohio.
About the Role:- As a Home Health Occupational Therapist, you will provide comprehensive skilled services to patients in their homes, as ordered by the attending physician.
- You will educate patients and their family members on proper use of equipment and techniques to improve range of motion, muscle strength, and coordination.
- Develop and implement individualized OT plans of care, making adjustments as needed to ensure optimal patient outcomes.
- Collaborate with the interdisciplinary team to provide coordinated care and minimize patient risk.
- Communicate effectively with patients, families, physicians, and coworkers to ensure seamless care delivery.
- Current license as an Occupational Therapist in the state of employment.
- Minimum one-year experience as an OT in an acute care setting, with home care experience preferred.
- Valid driver's license and auto insurance in your name as a driver.
- Ability to meet the physical demands of the job.
