Job Title: Occupational Therapist Home Health

We are seeking a skilled Occupational Therapist to join our team at Alternate Solutions Health Network LLC. As a Home Health Occupational Therapist, you will play a vital role in providing high-quality care to patients in their homes.

About the Job

This is a full-time position that requires a current license in the state of employment and a minimum of one year of experience as an Occupational Therapist in an acute care setting. Home care experience is preferred.

Responsibilities
  • Provide skilled occupational therapy as ordered by the attending physician.
  • Evaluate the patient's level of functioning and develop an individualized OT plan of care.
  • Teach patients and their families on preventative measures and good decision-making to minimize patient risk.
  • Communicate with the COTA on supervisory visit results and complete all clinical documentation following agency protocol and Medicare/Federal guidelines.

Requirements
  • Occupational Therapist with a current license in the state of employment.
  • Minimum one year of experience as an Occupational Therapist in an acute care setting. Home care experience preferred.
  • Valid driver's license and auto insurance in your name as a driver.
  • Capable of all physical demands.
