Job Title: Occupational Therapist - Home Health

Human Touch Home Health is seeking an experienced Occupational Therapist to join our dynamic team. As an Occupational Therapist, you will be responsible for providing expert occupational therapy services to patients in their homes, tailored to individual needs.

Responsibilities:
  • Conduct comprehensive assessments of patients' physical and functional abilities.
  • Develop personalized treatment plans in accordance with physician recommendations and patients' goals.
  • Provide skilled occupational therapy interventions aimed at improving mobility, strength, balance, and overall function.
  • Educate patients and their caregivers on therapeutic exercises, safety precautions, and injury prevention strategies.
  • Monitor patients' progress and adjust treatment plans as necessary to optimize outcomes.
  • Document all clinical findings, interventions, and patient responses accurately and in a timely manner.
  • Collaborate effectively with physicians, nurses, and other healthcare professionals to ensure coordinated care and achieve desired outcomes.
Qualifications:
  • Graduate of an accredited occupational therapy program with a current license to practice in the state of Washington, DC.
  • Minimum of 3 years of clinical experience as an occupational therapist, with a focus on home health care or skilled nursing facility preferred.
  • Strong clinical reasoning and problem-solving skills, with the ability to adapt treatment plans based on individual patient needs.
  • Strong clinical skills in assessment, treatment planning, and therapeutic interventions for a diverse patient population, including adults and older adults.
