Job Summary

Key Responsibilities
  • Monitor and coordinate healthcare services for participants, ensuring optimal health and well-being.
  • Institute appropriate follow-up and communicate medical issues to relevant healthcare providers and staff.
  • Document all nursing interventions and changes in participant care, maintaining accurate records.
  • Assist in training staff on personal care, positioning, feeding, and use of supportive devices.
  • Accompany participants to medical appointments and Day Programs.
Requirements
  • Valid MA Nursing License.
  • Valid driver's license and acceptable driving record.
  • Physical ability to perform the requirements of the position.
  • Good communication and computer skills.
